Working with Your Pharmacist to Manage a Chronic Disease

According to the U.S. Center for Health Statistics, a chronic disease is one lasting three months or more. Chronic diseases generally cannot be prevented by vaccines or cured by medication, nor do they generally just disappear. The goal for you, your doctor and your pharmacist is to control and manage the disease. These conditions can include asthma, arthritis, diabetes and heart diseases, among others.


Your pharmacist is trained to provide patient education and counseling as part of pharmacy services. When you pick up your prescriptions for treating your condition, be sure to talk with the pharmacist about your care plan for your chronic condition; the most effective way to take your medications; and any possible ... drug interactions that may occur, based on other medications and supplements you may be taking. Your pharmacist is your partner in helping you live — well — with a chronic condition.
